- (Nova series) Recent advances in airplane design have decreased the number of accidents caused by mechanical failure. However, the percentage of accidents due to human error has not declined. NOVA examines in detail three major crashes. United 173 ran out of fuel over Portland, Oregon and no one noticed until it was too late. Air Florida 90 was not properly de-iced and crashed shortly after takeoff from Washington, D. C. Delta 191 was caught in an unreported windshear on final approach to Dallas/Fort Worth. These crashes raise questions about the effects of human error, crew communication, the increasing use of automated equipment, deregulation and weather conditions on airline safety.
